Application Fees will be billed upon receipt of this application:

• $740 per 4,000 linear ft (or any portion thereof)

1. Proposed Route Map, including needed destination points

Equipment

Application Fees will be billed upon receipt of this application:

• $100 per max 20

The following Required Files must be attached to this application:

1. Location Map
2. Proposed Make-Ready Worksheet
3. Pole Load Analysis
4. Manufacturer Specs/ Design

A single application can be submitted for a maximum of 20 units of equipment

Wired Attachments

Application Fees will be billed upon receipt of this application:

• $1,000 for up to 50 pole counts
• $20 for each pole count above 50

The following Required Files must be attached to this application:

1. Route Map
2. Proposed Make-Ready Worksheet

A single application can be submitted for a maximum of 50 pole counts

Wireless Attachments

Application Fees will be billed upon receipt of this application:

• $100 per site application
• $413 engineering fee (where no replacement pole is required)
• $564 engineering fee (where a new or replacement pole is required)
